Responsibilities:
Conduct assessments of residents' social and emotional needs.
Develop and implement care plans to address individual resident needs.
Provide emotional support and companionship to residents.
Assist in coordinating residents' access to community services and resources.
Collaborate with the Social Services team to ensure the well-being of our residents.
Maintain accurate and confidential resident records.
Requirements:
High school diploma or its equivalent is required.
An Associate or Bachelor's Degree is preferred but not required.
Prior experience in healthcare or social services is preferred.
A genuine passion for working with seniors and providing them with compassionate care.
Strong communication and interpersonal skills.
Empathy, patience, and a positive attitude.
